if you open FC4 in a dissembler and force read 0x0000000001a7113e or 0x000000000009402f the Graphic driver crashes.
FC64.dll is actually the "game.exe"
FC4 uses only a loader.

I too have been suffering with these crashes with 2x980's in SLI in stock form and even when slightly under clocked. However I have now solved my issue completely and have been playing Far Cry 4 for two days now at max settings with HDAO+ without any crashes at all, so I just thought I'd share my experience just in case it can help someone.
I was using MSI Afterburner for overclocking and as soon as I closed the actual MSI window (which still makes the card run at overclocked values) and ran the game everything is working perfectly even with overclocked values of 1475Mhz @ 1.237/1.262v and an additional 400Mhz on the memory boost.
So if any of you are using MSI Afterburner this might be worth a shot, it worked for me, hope it helps!!
