...seems to be related to swapping weapons while ADS'ing. And for some reason it's more consistent when swapping from a magnified optic, but can still happen when swapping from any weapon while ADS'ing.

This really needs to be fixed, it's ridiculous.

I show the accuracy bug that's created by staying ADS's while swapping weapons, then show how it corrects itself after ADS'ing in and out a time or 2, then show me creating the a second time. I've had the issue when swapping from primary to pistol too. If you swap while ADS'ing the bug will persist as long as you continue to ADS, even through reloads.

I obviously haven't testing everything, but I have had the bug occur when switching to several different ASR's, SMG's, and HDG's.

Sorry, I bumped it since it was never added to the sticky list of know issues and I didn't want it to get lost since I've seen people complain elsewhere about accuracy issues that are likely related.
