Never happened to me, at least not until now
Maybe they're critical of the NPCs or the player's computer or maybe server lags
Players with bad builds (and crying babies) who didn't adapt to The Division 2 gameplay
Occasionally occurs: some NPCs, sometimes, (hello Hyenas red bar) think they are Hunters
It ALWAYS happens, whenever I die it is the fault of the game that is cheating ... damn developers
Lag/server not being able to keep up is definitely contributing to this. What you see on-screen is not always what's happening on the server. See also: people complaining about cheaters (no: again, what you see on-screen is not always what's happening on the server).
I suspect even the level scaling bugs we talked about some months ago are due to this.
Same for........drum roll......hit registration issues.
All these issues boil down to crappy servers/netcode, even when playing solo (always online design).

The original quote says "stupidity" at the end, but I think this version is a bit more fitting.
When going in an elevator the re spawn is different for everyone - some spawn right away players like me must wait a painfully long time to spawn as the game trys to catch up - you can hear the battle going on but your still waiting to spawn. Back ground is white when spawning and then comes into view. Skills sitting idol not taking commands - button for the vendor take a bit of time to appear - lots of evidence of a lag but obviously not for all players.
