On ps5 I will throw the bomb and it won’t place, and show that it’s in active cool down, won’t let me use it for the rest of the game

After doing some research, I guess it tends to happen most when you try to use it but get either interrupted or killed before it truly activates, causing it to be in some sort of loop.
