Yes, right as usual, KnightGhost has nailed the issue. Each district in the map has a distinct level rating. You can see the level at the street level when you change zones. Or, you can also see it from the map view. The lowest level is near where you come in and goes up as you travel straight up the left side. Then, you'll go back to the bottom center and work your way to the right side of the map. It keep getting higher level requirements, culminating in the far north area where you fight the final mission story-wise (General Assembly). Generally you'd be level 26+ at that point, and most are 28-30 by then.
I got a new copy of the game during the free period. I have the benefit of 10 play-thru's and 4k hours at this point so I know the game. I tried to rush a bit and while the challenge was fun, I had skipped most side missions. This had me behind the XP/Level curve. I finally hit a point that I couldn't win the missions when the NPC's were 6+ levels above me. The street rabble was still fine, but not the missions, even on normal difficulty. So, I stopped progressing and went and finished some side missions to give me the XP needed to continue on.
