A couple are edited, though only for contrast/brightness, and occasionally for colour balance to try and make sure the image appears perfect on as many monitor types as possible. Otherwise everything else is done in the game. As to how, it might help to know that of the 422 hours I've spent in this game, almost 100 hours of that has been spent using the photomode. It used to take me a good hour or so to set up a shot, now it takes 2 minutes or less. It's just a matter of practice.

I've been experimenting recently with using Photo Mode while in combat, and these three are the best shots I've gotten so far. The first one I took just as a Hyena Engineer fired his shotgun, and managed to catch the shotgun pellets in mid-air.
I've also been experimenting with the photo effects available in Photo Mode, such as Exposure, Contrast and Focus. The last three are from the Jefferson Plaza mission when I replayed it for a Daily. Holy hell, the entire Jefferson Plaza mission area is so gorgeous and well designed!
