Holt repeating a phrase from the end of a mission, used quick travel while he was speaking.
I went through several missions to get it to stop, even got him killed,(only stopped it until he was revived)
Thought I'd fixed it after 3 hours and several consecutive story missions without dying.
But this morning when my boy loaded the game again, it was back, Everything else in the game is fine.
Is there a possible fix for this, or does he have to delete his game and start again?

What's the exact phrase he keeps repeating? If it's about returning to the regular schedule of freeing Bolivia from the Cartel, then you didn't complete "Operation Oracle" properly. With Operation Oracle you're supposed to go back and record the engineers phone calls after leaving him at the safehouse - there's a couple of other missions with similar issues (which is why I asked for the phrase he's saying, ad it will tell us exactly which mission caused the issue for you).
Simply go and complete that mission again properly and it should fix the issue.

If possible, would you be able to send us a video showing this occurring in game so we can take a closer look?
