I think it was circa 1.6 update, they took a lot of emotes and some clothing and instead of being able to earn credits in game, they became micro-transactions that can only be purchased with real money. As long as you have all the sets offered through encrypted keys, then you won't get anything new. The way to verify this is to look at the Mark Caches and it will show you if you have them all. I will say that if you have them all but didn't get the "keystone" item, I've had to click each item in a collection before it would show up. I hope that makes sense as it is hard to describe without pictures.
Myself, I have ~75 keys on one account and ~150 keys on the other as there is nothing to buy with them. That said, I think most of my characters still have the original look from when I set them up. Each of my characters are unique so I know which character I am playing as. I did this mostly to be able to know which loadouts are saved and not so much for vanity purposes.
Some cosmetics you can only get by doing certain things. For instance, the Banshee outfit is given automatically when you hit max level (40) in the Underground.
Personally, I will never, ever, ever, buy clothing, emotes, masks, etc with real money in ANY game. I have thought about it a lot and if there was anything I'd ever consider paying for a micro-transaction for a game might be stash space which is essentially why I initially bought the Ultimate edition of TD2 before they changed their minds as did I.
