Because of submissions on consoles everything is planned out ahead of time.
Games have to go into submission, which is why day 0 patches exist.
The devs send the game to submission, but once in submission you can't change anything. If you make a change to a build, you have to pull it from and resubmit it. Instead devs work on day 0 patch, which fixes issues found by testers, while the game is in submission (usually a month or a few weeks before release to allow the production and shipping of physical copies).
Then Day 0 patch goes to submission (a week or two before release). Again, devs can't change the patch once it's submitted, so they start work on Patch 1 or 2, which again mostly fixes issues found by testers pre-release and maybe a few issues found by people post-release.
Patches are always a few weeks to a month or two behind whatever might be getting reported to the devs at the moment.
You don't have to take my word for it. I was just a tester for 5 years and this is how all the patches worked on all the games (many of which were AAA) I tested.
