This happened often enough that i'm sure others have also reported it, but here are two that i was able to capture, and one more that i wasn't but happened repeatedly.

Stuck in the ground (happened to me twice and once that i saw to a friend while playing co-op):

Another i wasn't able to capture was, whenever i tried to sit in the back of a buggy, it would transport me to another part of the map. usually into the danger drone zone.
