Enemies blind until detection meter fills, can't get suspicious from a good glimpse

Enemies are functionally blind unless the detection meter fills completely. Even if they get a really good glimpse of you they won't actually get suspicious and do a short search. This makes them feel pretty braindead while you're undetected.

Might be platform related? I'm on Xbox one and playing on the second highest difficulty (forgot the name).
Sometimes they just barely see me and go on suspicious mode.

    Platform Related

I'm also on Xbox One S and for the most part on easy and Hard difficulty the enemies have been able to detect me and even do a soft search of an area. At a distance they can struggle but terrain and player position can play a part in that.

@ArgimonEd They must have heard you or found a body.

@Parthanaxxxxx The only way they'll perform a soft search is if they hear something or find a body.

I just tested this against Wolves on Extreme difficulty. If their current alert status is at absolute zero then even filling the detection meter to 95% at fairly close range will not trigger any suspicion. Just because a red eye appears over their head doesn't mean they're actually reacting. The only time they ever reacted was when I moved close enough for them to hear me.

I'll try to see if it's different when they're already somewhat alert. But even if it is that's still a problem Ubisoft needs to address.

Edit: Testing against alert enemies was a bit trickier but it seems to be the same situation. I don't think they ever shifted their attention unless I allowed the detection meter to fill to 100%.

No, I was to far away to be heard and no body, but still in reach of their field of view, the detection meter started to fill, but I gone prone, they got some Yellow markers on top of their heads and started to walk around.
And I wasn't moving, I was crouched and doing recon to mark all targets in the area.

You haven't run into the guy with the rockets on his back. He can see you a football field away.

They don't 'forget' you were there per se, they think you've moved on so they stop looking for you. But yeah, you'd think they'd remain suspisious, on low alert or something
