Or when you win or escape a fight with very low health and are going to a zone to heal but a teammate hits and kills you just to revive you for the points, that shouldn't go unpunished either.
For every friendly kill, they should lose points and be sent back 1 feat.

There is unavoidable friendly fire, like bulwark countering a bunch of opponents and a team mate caught in it. I don't think all friendly fire should be punishable, especially since there are moments when it does worth the suffered damage. Ballista shots can also be accidents, especially if there were enemy players nearby, althought if you care at least a little, you will never hit anything you don't want to.

Intentional and out of ignorance friendly fire, on the other hand, should be really punished. Maybe if the system would record how much damage you dealt to your team mates during a match, it could easily tell this, and a griefing report would be more effective.

some friendly fire is also quite good in breach. Had a few team mates myself included who would team kill a low health mate on purpose when no opponent is around and revive them on the spot to half health. No need to mention that this saves a ticket that could easily been lost on the way to the healing zone or while trying to get an execute on a captain with an aggroed minion next to it.
