I think I might understand the vanity item drop bug - and it involves the store.

The Vanity Drop Bug from my perspective:

The equipment dyes found in the orange crates in the PVE open world do not drop for me regardless of level, world tier, etc. I have ran through and collected as many crates as possible to no avail. I still receive the two gear pieces from each crate, but dyes do not drop.

I have tried the following to amend the issue with no success:

    - At world tier 2, joined another player`s session to collect crates.
    - Waited for the loot refresh timer to reset.
    - Made a second agent character, leveled said character to appropriate level to collect cosmetics, crates, etc.
    - Made sure my game properly installed updates and is up to the current version of the game.

Before Title Update 3.0, I had collected 3 out of the 10 dyes found in crates. Post title update, they no longer drop along with other vanity items such as clothing.

    Video of Bug:

    https://www.youtube.com/watch?v=6ue_...ature=youtu.be

In the video, I show one one of the crates that are suppose to drop an equipment dye. I then briefly show my character level and all the dyes I posses currently. Finally, I show that dyes do not drop for me by showing the crate contents.

    Possible Cause:

I believe this bug could possibly be affected by the apparel store and newer items being released. Once you precede a specific amount of items (collected from activities, store, events, etc.) the game`s system assumes you own all of the items that could be collected from the open world so they would no longer drop although that is not the case seeing how I am legitimately missing items.

Now this is completely hypothetical and I have a strong feeling that it could more complicated than this, but hopefully this can bring up discussion and work towards a solution to fix this bug. I am a solo player and am unable to participate in a lot of content in the game so collecting and customization are some of my favorite pastimes in the Division 2. Hopefully this gets fixed soon

Thank you for the video and detailed report Spanky_Actual! I will pass this on to the TD2 team for investigation.
