It seems like a pretty rare enough occurrence but I have run into a few breach matches where my teammates would rather purposefully deplete our lives (attacker side) when we are losing a match instead of just leaving; I assume to avoid the leaving penalty. Not sure that much can be done for those that run into enemies and purposefully die, but the other day I was in a match where two teammates left the game so it was me and one other guy along with two bots. I was managing pretty well on my own given the circumstances but at one point when I died and spectated the other guy, he was running from spawn and B-lining right for an edge to jump off and kill himself. I revived him when I could to save lives but he kept this up for almost the entirety of the first phase.

Even with that disadvantage, we broke the gate and made it to the second area before he depleted all our lives. Again, I was handling it pretty well with the bots. It was a match that we could've definitely won. This time, every time he respawned, he would run to the first area and bleed out of bounds where he couldn't be revived. It was frustrating to say the least that I could report him but he was still in the game depleting lives for no reason. Of course we lost by breaking with barely any ram damage.

This problem in particular I think could be easily addressed. Players that die from out of bounds damage 3-5 times should be booted from the game with the leave penalty, or out-of-bounds areas should be blocked off after all players enter the next area. There's very little reason or incentive to go out of bounds after attackers break the gate. It just makes it too easy to grief your team.
