I'm also curious about Death From Above and when you grab a dude. Do the same rules apply there.
I've seen Sam break their necks in both situation, but I've also seen him KO them, I should check the status of his weapon.
EDIT: Doesn't seem to be the case.
well jazz there is one death from above
that if you on a wall and do it
you will jump and kick his back (see the video about the suit for the code unlock)
that can be a KO
but knowing how a body works
if the neck slaps back hard it could break and kill him.

I really don't think we can take Redding at his word. A few months ago, we were told that being empty-handed would make you non-lethal, and I thought "hey that makes sense; gun=kill; no gun=no kill". Now we see that's not the case. What, I have to ask, is the correlation between a two or one handed weapon, and lethality? It really doesn't make much sense.
The Redding believes the damage is less than lethal isn't good enough. With "Kill" coming up regardless of equipment, no way to see thermal signatures, and enemies unable to revive "unconscious" allies, then it only seems logical to assume that the man you just ganked with your shotgun is dead.
What would be helpful as a tell-tale sign, would be a forming pool of blood under all dead bodies.
