I experienced this same issue on PS4 Pro.
Got the game today, downloaded and installed all patches/title updates before starting. Created my agent, played and completed the first Theater mission, and then logged restarted my PS4 due to framerate issues.
Upon loading up my game, my agent's appearance had changed completely. I'd like this issue resolved before I spend any further time creating and customizing new agents. Thank you.
UPDATE: Restarting the game fixed the issue. My agent's appearance loaded up correctly after closing down The Division 2 and launching it again. (PS4 Pro)
@fenrir280 Do you still see you agent's customized appearance on the title screen before loading your game?
And if not, did you complete anything in-game using the "default" face applied to your agent?
If you do see your custom agent on the title screen, I would assume that Massive can fix the game to allow your agent to load correctly (though I'm saying this without having seen the code).

@Fenrir280 I wasn't sure if the agent's current appearance was saved as part of the game save. I restarted the game immediately to see if I could get my agent to reload and didn't want to do anything else for fear of overwriting anything but I wouldn't expect everyone to do that either. Hopefully Ubi can do something to help: I'm sure this same issue is affecting others out there.
