Game is loading something from HDD permanently, all the time. Even if i'm staying in place for several minutes.
When i'm moving through city, loading of objects (and sometimes even enemies) is taking too long, leading to invisible objects and enemies.
Game perform relatively well, if installed on SSD, yet i don't see any mentions of SSD in system requirements.
Is that considered as issue? Or game is fine and this is my PC problem?

From the description of the issue, it sounds like the read speed of your HDD is quite slow or there is an issue. Could you please confirm the make/model of your hard drive and anything that might be useful about it?

Had the same problem on my Seagate 500 GB HDD 5600 RPM. Restarting my PC fixed it however. The HDD is pretty new (less than a year).

System and Uplay+Game installed on SSD, but i have constantly freeze due use of my other HDD.
Cant fix this with restart. Cant remember this problem in private beta. For now game is just unplayable for me.

My HDD is HITACHI HTS721010A9E630
The rest of Ubisof game (like The Division, Far Cry 5, Far Cry New Dawn, Assassins Creed Odyssey, R6 and For Honor) are working fine on my laptop. I also didn't experience such issues during alpha testing.
