There isn't any kind of offer blacklist, so you don't need to worry about that. It could be that they haven't been unlocked. What level are you on for PvE mode?

Yes. I believe it's Episode 3, stage 3 that will unlock everything. Once you pass there, that will signal to the game that you have progressed far enough for everything to be shown to you.
