Make a ticket on Ubisoft support. Took less than an hour to resolve (litterally 47min). I did attach a screenshot of my daily login to the ticket as well. See below.

Sam's Axe is still locked after 3 days of Daily Logins. Today's Daily Login Screenshot Attached.
Me Too I've been trying for months now.. I tried loging in multiple times it detected that i logged in but still didn't give the axe as a reward... i even delete my saved data and start a new fresh save with a different character but still nothing

I guess it's true you want what you can't have. Or the more out of reach, the more you strive to bridge that gap.
I've had fun with Sam discussing a whole Skynet/Genesis (Terminator) apocalypse, I've responded with stupid responses like, "I volunteer to help serve our robotic overlords!" and he has made me laugh a few times. Stupid but since it's not something I triggered, Sam demonstrates a personality even at its lowest form. Kudos ubisoft I like Sam.
