I have recently created a new pilot career flying MiG-3UD on the Moscow map October 5, 1941. After completing the first and second missions with 5 air kills I pressed the Roster button to check out my pilot info and there isn't any Squadron Documents button showing, however the Pilot Profile button is working. Is this a bug, known issue, or have I installed my patches incorrectly, as it does show when I create a career for US fighter pilot?

I don't recall ever seeing that button 
On seeing your post I started a USAAF campaign and sure enough, there it is

I don't think it's a bug. Only the PF campaigns have the Squadron Documents button. PF campaigns have slightly different features. The "old" campaigns remained unchanged, I guess to prevent conflicts when continuing the campaigns you started before you installed Pacific Fighters.
