But, I think that on Extreme Difficulty - when your squad gets killed, they should have a cool down timer, and only be able to rejoin the players Squad — IF the player goes back to a friendly outpost, and selects a ‘rejoin’ option for the squad to spawn.
I hate the way the Squad are practically immortal.
There should be a penalty if they get killed.....

As Ghost416 told you, the AI can actually die if they aren't revive in time, same as you, it works the same in GRFS.
I get your idea, GRAW 2 and previous games allowed your teammates (if they weren't revive in time) to have "perma death" however, this game works in a very different way and the structure is different, we don't have reinforcements in this game and Future Soldier, so if one of them bleeds out you fail.
Maybe you haven't notice it because one of your teammates revives him or because you left the area and let them behind which will caused to get revive by their own.
