I mostly play solo. I just recently started playing and the game is empty. Perhaps, most of these complaints are mitigated when you play with a group of players.
Enemy Accuracy
When I fire my weapon, it spray is all over the place. I have to put effort into controlling it, and even still my accuracy it's poop at times.
When the enemy fires their weapon, they have stupid accuracy. An enemy could be running in the opposite direction with their arms strangely contorted backwards and their bullets hits. Why am I handicapped? Some enemies can down out before they're even within your range. The same accuracy mechanics that affect me should affect the npcs.
Blind fire is pointless. In most shooters, shooting blindly from cover is a strategic decision. You sacrifice accuracy to mitigate damage. However, in this game, blind fire is the same as standing in the middle of an empty street. The enemy can down you from simply shooting at your hand with ******* accuracy.
Random Spawns
Enemies are bullet sponges. When you're in an engagement, the last thing you want is to let an npc get behind it, so you pay close attention to the enemies movement, but it's pointless. This game likes to intentionally spawn enemies behind it when you are in the dark zone or open world. It's cheap, not difficult. I cleared my back. I didn't die because I wasn't watching it. I died because a random npc spawned behind me and down me in 2 or 3 shots.
Grenade Spam
Grenade. Grenade. Grenade. How many grenades does the enemy have? They will light up an entire area with grenades alone.
Rushers
Why do shotgunner, baseball bat wielders, and such have more life than most enemies?It feels cheap to get killed by a shotgunner after you barely managed to down the first one. Most of the time, you're **** out of luck when facing the second shotgunner. Since you had to switched between weapons to kill the first, you don't have enough ammo to kill the second. Trying to reload while running from a shotgunner that can snipe you is hard.
Shotgunner have ridiculous range.
Exotic Drops
Today, I saw my first exotic drop. I was like, "Hey, my first drop," but it disappeared as soon as I saw it, because I apparently have to compete against other players for the drops. I was playing with a random I was matched with. I thought we all had our own unique drop. This mechanic is dump.
This is more I wanted to express, but I don't really like to write long posts. I feel this game is flawed on a very fundamental level. I know changes won't happen since the Division 2 has been announced. This game will be abandoned soon, but these are my thoughts.

1- What rank are you ingame? and what tier are you playing in?
2- Accuracy:
-AI Are balanced for their world level.
-Each weapon has a range stats, AI utilize that.
-About AI running in the other direction and hitting you, i think i noticed that couple of times, but i rarely see AI shoot while running away.
-Blind fire can be upgraded with some talents and attachments, and ability of the AI to shoot your hands in blind fire is quit realistic.
3-Random spawn:
-Enemy are actually less sponge that before, trust me, you should be happy your not playing a year after launch.
-You are correct with some of the encounters with AI spawning behind you ingame, but am pretty sure there is a distance which they cant spawn in from you.
-If you die from 2-3 shots in any world tier rank, you really need to increase your Armor and Stamina.
4-Grenade spam
-You need to master the double Space bar tactics to avoid grenades, AI as i hope you know get an icon of top of their head when they are throwing a grenade, That's enough time to avoid a grenade.
-Grenade is a vital part of an infantry combat, its used to flash you out of cover by the AI or to make you stop shooting them.
-Depending on the veterancy of AI, you can make the AI drop their grenade by allocating them with disrupting damage (just shoot them enough to make the grenade fall on their feet, or grenade them first).
-If you hate grenades that much, get the final measure gear set.
5-Rushers
-I Hate them as you do
-Actually Rushers have less health from other classed except "Recon"
-Looters Rusher have a Shield, just don't shoot the shield.
-Master the "G+Left click" tactic to either disrupt their run, slow them down, and hold them in place, you have many grenades options that you should use
-Shotguns in real life have quite a range, depending on the ammo of course.
6-Exotic Drops
-Un-grouped Player in DZ can take your dropped loot, but in Group and all Pve Games, All group members can pick up the dropped loot.
You are correct mate, the game has a lot of problems, but the Devs have fixed a lot of issues with the game thankfully. Nothing is perfect in life.
Hope Division 2 fixed a lot of the stuff wrong with the current game and add more. but until then you have to "Improvise, adapt and overcome".

1.3 Underground was fun when it took a bit of thought.
if anything its been made to easy because of people complaining, the simple answer is make more levels the tiers are pretty pointless separating games, better off just having
Hard, challenging, very challenging, heroic and legendary on all missions..
I do agree that the game gets much easier when you past level 30 and your gear set above 270. Win fights against the AI even in larger groups isn't that hard as before, but...
(Enemy) Accuracy: I think is an issue. Rioters using pistols seem to be very good at aiming even at long range. They hardly ever miss the target, they don't do a lot of damage but it is just not very realistic shooting handguns like that. What strikes me most is some of the Elite Bosses that run and bounce around like they are high on drugs, running sideways, continue shooting while moving and never seem to miss. Very, very unrealistic. The same goes for the heavy gunners that don't seem to have any problem keeping their aim on the target, even tho they use an LMG their damage output is outrageous. What i do experience a lot, is when shooting at a target, hitting hard on elites for instance all of sudden your aim starts to deflect from the target. It feels that you are only allowed to do so much damage.
Random spawn: This is an issue, and a big problem. There are places on the map were you can walk without seeing an enemy, yet if you move another inch you are right in the middle of them, and they start shooting as soon as you see them. Not sure if this is related to performance of my system or just bad programming of spawns. Next to this I also had situations were if you do pulse, you see 3 enemies in area, but when you start shooting they are all over the place and certainly more then the initial 3. Besides this it also seems that, the faster you kill, the more enemies are thrown at you and spawning all around you.
Grenade (spam): The use of grenades I guess is fine, AI can use launchers that can fire lots of them and fast. But what I don't like is that when in cover you are affected by them and your character gets up first to take the full blast. Isn't very realistic and makes use of cover actually useless, and this also happens when you are standing behind walls or corner of building and with the explosion on other side. You always take full damage if you are in the red circle no matter what objects are obstructing the blast, they are just not there.
Rushers: Personally I think these guys are plain stupid but very effective for the AI defense wise. While running towards you with their shotguns they don't miss and this also applies at long range. If you try this yourself you can't get a straight shot out of a shotgun when you are moving. With most shotguns they only do damage if you are standing still, and even then you need to take several shots when at close range from target. Not sure it is simply a miss, or bad calculation of damage and critical hits, but it you are shot by one at close range it hurts for sure.
Exotic drops: I don't see this as a problem, I did get most of the gear and complete sets. It only takes some time, you don't get a complete set in like 1-2 hours. As for exotic I can also say that I did get many duplicates, the gloves, backpacks and some weapons just keep dropping all over the place, while others are actually quite rare and exotic.
AI behavior: This one I would like to add to the list, because I do think this is really annoying at times. For instance if you ambush the AI, they tend to zap into cover, run really fast or re-spawn at a location that is further away from you and you have to aim again. Same as during gunfight, they always need some space between you before they start shooting. And when they are out of sight of your camera they also move or zap to locations in short time spans that makes it is seem impossible for you to manage that distance yourself. Personally I would have liked to see the AI more prepared and aware of what is going on around them, sometimes you are literally few feet away but they don't react and keep walking. This also applies to JTF, they all need glasses.
