It's some nonsense they added in season 5 where certain gear can have one final level (negligibly increasing stats) added on for 1000 steel after its level has been maxed out.

This is a symbol for refined gear. Refined gear will give equipment a stat boost above the maximum. However, refinable gear is rare and can only be looted at Reputation 9 and up. Every reputation level after 9 will increase the chance for refined gear to be looted.
