so for instance let the player actually defeat Joseph Seed (kill him) and maybe rescue a few prisoners

Because you would be defeating the head of the cult and with the new ending there was no nuclear bomb or character stuck in a bunker with Joseph there is a way to re enter Montana with the world in-tact and all progress intact.
The Sheriff could say something like, "Good job rook, with your help we have defeated the bulk of the cult. Feel free to stay here with us and help us take down the remaining cultist outposts scattered across the region." At this point the player would go back into their game with any remaining side missions and outpost left to liberate and have the option of resetting outposts as is already in the game- then an option could come up like it is in Dark Souls 3 to start new game then or at any time later. A person could finish outposts, reset outposts until they are ready to hit new game plus in which case you would simply start the game anew with your achievements, weapons, etc intact- More perks, hardy enemies, extra challenges and the like would be welcome here.

Over all I am saying that the optional ending wold keep Montana intact as far as looking beautiful (not having the nuke) it would also make the player feel good that all of the work: rescues, liberations, roster additions, weapons, achievements meant something and allows them to then free roam in a still thriving and liberated (or not) game state!
Where do I vote for THIS option, excellent post - (although I have to admit that this scenario contains far more common-sense than the numb-nuts at UBI could probably fathom because from all appearances those folks could F#$% up a wet dream)
