Just to preface, none of this is relevant with the current “Resistance Meter” system in place, as it reduces enemy encounters the more you play. So technically, the FIRST adjustment should be to “reset” enemy spawns/patrols.
3 "practical" adjustments for a Survival Mode
- Limited Health Regeneration
- Limited by either NO regeneration, or up to 20-50% health regeneration. Medkits become more meaningful and forces management over long-term.
- This adjustment would require adding an entirely new mechanic, so it’s not quite as practical. When out of medkits, and assuming health regen is <= 25%, utilize a field medic mechanic which allows healing up to 50% of your total health.
- Limited Fast Travel
- Fast travel from anywhere on the map is no longer permitted. The player must be located at a captured outpost to fast travel to another location.
- A resource cost could also be applied for fast travel (i.e. money, ammo, medkits). Consequently, fast travel costs between outposts could be mitigated.
- Limited Weapon/Equipment Inventory
- Cut “deeper selections” within weapon wheel slots—One weapon/item per slot.
- Firearms have 3 designated slots (maximum): One slot exclusive for sidearms, and two slots for any one or two-handed weapons.
- Melee weapons have 2 designated slots: one slot exclusive for unarmed/fists and another slot for a single melee weapon (e.g. baseball bat 9/9).
- Throwables have 3 designated slots (again, no deep selection option). Limited equipment options creates choices for various loadout combinations.
Obviously these aren't "new" ideas, but they were the most practical in terms of not having to implement anything too complex. I have other suggestions, but they range from inconsequential tweaks like limiting specific UI elements, to far more in-depth changes, like redesigning perks, adjusting spawn rates/quantities of health/ammo, guns-for-hire balances, implementation of an Attrition/Stamina system… just realized no one is listening.

I think you can play like this without any mods. Just use some self discipline. Don’t increase your health skill, don’t fast travel, and don’t increase your inventory options. It’s actually not all that difficult if you stay off roads, are quick and accurate, and don’t get yourself surrounded. In fact, I reckon you could finish the game with just the bow and a dozen arrows if you’re smart, quick, and accurate.
By far, the biggest cheat in this game is the red indicators showing where enemies are located, and the outlines that show their facing and stance even when out of line of sight. I’ve turned weapon tagging off as a result, but the game still seems to insist on showing me where everyone is and what their doing. It’s horribly easy with this “feature”.
