I had barely travelled out of John Seed's area before this had happened (still haven't). Briefly wandering into Faith's region for a second before turning back (that was much earlier).
As for restart I don't think this helped. But I will double check next time I play.
Still an enjoyable game though, when I don't suck at it.
Here's when in the sequence of doing various Holland Valley missions that the Valley Armed Convoy side mission disappeared:
Went back to a hard save: the Holland Valley convoy side mission was no longer active (it's still shown in the journal however) following The Confession completion (and when I automatically got the Revelator mission). It most likely disappears completely from the journal shortly thereafter.
The above sequence Holland Valley missions did not include doing Wingman nor Air Raid. It did however include liberating Falls End, followed by Widowmaker.
An update: We are investigating. Please create a ticket with your save file if possible.

UPDATE: This takes the cake - the Ubisoft customer system doesn't accept its own created save file(s).
Error message of "you can't upload files of this type."
A search in the CS system produces no result for what "type" of file Ubisoft is expecting.
