The Life Upgrades are the same on PC, PS2, Xbox, etc.
In the normal progress of the game you will find the mask. (You do not have to search on your own for the mask.) The game will play an important cinematic scene.
Whenever you are in the Hourglass Chamber (between the Central Access Hall and the Throne Room), look at the circle of nine icons in the floor. When you find each Life Upgrade, a certain icon on the floor lights up.
Within this discussion topic I included a cross-reference of each floor icon with the Life Upgrade.

You will fight the Empress at the end of the game and see an "alternate" (or default) ending to the game.
In order to properly earn your way to the "preferred" Dahaka ending, you must earn all nine Life Upgrades.
