Relatively new to the forums and the game (PC player).
So this is not so much a whinge, but a legitimate and logical question as to the current state of the match making system, and how it is operating.
After noticing the constant bad match-ups I was receiving, I decided to do some googling on the topic.
I found quite a number of threads/posts, but many being older, I determined that issues that were 6-8 months old were likely no longer valid.
But I've also come across more recent posts on the topic, such as this: (1 month ago)

So my personal experiences are as follows:
I am constantly matched up with players 20-60 reputation levels above me, often in our countries, who are generally far above my skill level.
Usually this occurs because the match making has taken so long, that all the criteria have extended to "Wide or All". So my assumption would be that there is simply not enough people attempting to play at this time, and it has subsequently matched me with the only suitalbe player it could find.
This generally leads to an extremely negative playing experience. Most of the time by me (getting my *** kicked), but I expect by my opponent sometimes as well, as not everyone always likes wiping the floor with their opponent, and I fully realise I am not providing them with a particularly challenging experience.
Also please understand, I'm all for learning the game and wanting to get better. I do try to analyse why I lose, and am noticing meta trends and the like of how people are playing and beating me, but generally you prefer for this to be a 'gradual' increase as you slowly get better. This is not the experience I'm having.
So I decided to also try to do some tests on this front, using some friends. We usually play together, but decided to try to queue up for some 1v1's.
We all got the game around the same time, live in the same country\region, we are all very similar in skill, and our reputation levels are no more than 1-2 levels apart in all cases. We all have Green/Open NAT's.
This is when I realised something must be broken. While there were the very rare occassion we would be matched up, for the most part, we never were.
We would be on voice comms together, we would click the 'Play' button at exactly the same time, and watch as slowly the Region, Skill and Reputation all increased to either Wide/All, and consequently matched us with ridiculous opponents almost every time..... What the actual f*** is going on?
It's almost like we are standing right next to eachother, but as far as the game is concerned, the other player simply does not exist.
I can replicate this, over and over again.
Now as much as I would love to be able to play proper Duels with my friends while also being able to obtain some level of progression on my character, that's not really the main point. I would be just as happy to be consistently matched with other players of equal skill & rep, if they exist. If the player base happens to be low, then I understand the need to increase the search/match criteria.... but if there is a player RIGHT THERE, also looking for a match, who is of suitable region/skill/reputation, why in gods name are we not being matched together?
There is a fundemental issue here, and one that needs to be rectified. As it stands, I simply don't want to Duel PvP anymore. The experience is too negative, and I'm not enjoying myself at all. Through this, I fully understand I simply make the situation worse, as the lower the concurrent matching player base becomes, the worse the match making skill/rep seperation becomes. I should not be matched with a 63 Rep player, when I know there is another player the same Rep as me looking for a match at that very time.
Perhaps if the match making cannot be fixed, something as simple as a server browser can be implemented, where I join someones lobby, while easily being able to view their Skill/Rep/Region before doing so, determining whether I will have an enjoyable experience or not.
Custom games are not the solution here, as there is zero hero progression.

Further to this, how do the dedicated servers impact the match making?
ie.
Will we get 'match maked' to areas outside of dedicated server area? (Is that what "Region" means?)
If so, what determines what dedicated server we use?
For reference, I am in the Oceanic/Australia region.
It certainly appears that way, but you would assume that is what the Region/Skill/Reputation values are doing as they dynamically change.
Attempting to match you with players that meet those criteria......
The problem is, it rarely seems to actually happen.
I would put it down to low population & lack of matches, if it wasn't for the fact I had access to friends to conduct specific tests, that prove these criteria simply aren't being matched at the time.
Matchmaking in 4v4 is pretty terrible. My team gets steam rolled on a pretty consistent basis. I'll have 6 or mroe caps with over ten kills and under 5 deaths but my team will have a grand total of 0 caps to share and like 2 kills between the 3 of them. I guess it tries to mix people with others who can compensate for them. But even I can't carry that much
