I went through this before; let me link you the thread where a representative assisted me:

Basically, you have to contact Support, submit a ticket/case, and provide evidence videos/footage. Provide them as much detail as you can. The representative gave me the link in the thread.

Also, Ubisoft Support and the investigation team (the guys who will study your evidence) are two separate groups, and Support cannot force details out of the investigation team. The investigation team won't communicate details with the players, so don't bother asking if they proceeded to ban the person you are reporting.

That said, the entire process isn't complicated at all.
