How about a Tribesman faction? Maybe Zulu, Hun, Apache (or Comanche?), Aztec, Maui and Dacians.
Maybe the smaller factions banded together after the disaster that occurs in for honor to form their own faction in order to compete with the much stronger main three.
Somehow Apollyon (or a new version) could lead them in order to make a new faction of 'wolves'. Maybe even a new story campaign?
Technically when we are fighting in PVP/PvAi we are fighting in the war that ensues after Apollyons death and the events in the campaign. When we see the 3 leaders at the end discussing peace, that’s 7 Years after her death I believe. Apollyon won’t make it into the PVP aspect of the game as she’s dead, and there are issues with her Armor clipping or something of the sort.

Something else that will never happen is the introduction of a new faction. This would spoil the fairly effective 3 way battle that goes on and likely result in 2 factions against the other 2. It would also require the team to make 6 new classes for said faction presumably.

I'm personally against having Apollyon as a playable character in PvP. It would lessen her overall image and impact. People will meme the crap out of her. And moveset-wise, she will be a literally overpowered hero in PvP. She has insane recovery times and an insane stamina pool. Also her armor isn't PvP-friendly because it has a static, fixed look and detail. It cannot be swapped/modified/customized. On top of confirmed to cause clipping issues if they make this armor available for multiplayer, it's been mentioned before.
A new fourth Faction is nice, but until they properly address the lore side of things, re-push the narrative into a much deeper hole, and hire a writer to take Ari's seat, I'd rather they don't introduce any new Factions now.
Upon Apollyon's death, from all the chaos she's been spreading until her death, three Factions remain at eternal war for 7 years straight. This 7-year war is the age of wolves; post-Apollyon era, but all of it driven from Apollyon's actions. So yeah lore-wise it wouldn't make sense at all if Apollyon makes a reappearance anywhere.
The lore may seem insignificant but only because no proper effort has been put into it. Ari said so herself. She had so many ideas but much of her thinking has never been codified. But don't worry, the potential is there.

The 'wolves' are the people Apollyon spared in her invasion and recruitment campaigns, effectively butchering 'sheep' and recruiting 'wolves', a.k.a the worst criminal filth who'd never take a second thought at plunging a dagger in your back the moment you divert your attention. So according to how Apollyon acted all this time, all three Factions are now mostly occupied by 'wolves', constantly killing each other.
These wolves, are us, the playerbase. The Great Cataclysm happened 1000 years ago in an alternate dimension, and ever since, there has been the Millenial War for survival and resources, up until Apollyon surfaced, a thousand years after the cataclysm. There could have been peace but then Apollyon happened and she re-erupted everything into total war. Figuratively speaking, For Honor represents the present day, and we all are Apollyon's wolves who surfaced after her campaign. Upon her death, all chaos erupted, starting at Blackstone Keep, where all three Factions met hap-hazardly. And are now fighting each other without really knowing why.
