Glitched Hero menu notifications:

*Centurion since release of S2. Keeps saying there is an effect that I have unlocked, but there isn't when I look.
*Kensei since mid S3, but recently Orochi. This was due to getting disconnected from Ubi servers while dismantling a piece of gear from the post game loot screen. The item I deleted won't be in the inventory, but the notification of a new item will persist. Now my Aramusha has the bugged notification as well.

Soldiers staggering during kill chain:

Every character I play besides Shugoki get staggered mid kill chain. This has been occurring since S3 for me.


"Faction War state unavailable at this time":

This has been happening periodically since the seamless maintenance a week ago. The map resets to 29/29/29, with no faction leading or trailing.

"Error Deploying War Assets":

This sometimes occurs post game where I will not be able to place assets on the map.
