I'm on xbox and have seen issues of shots be wildly off target. It happens with all guns it seems just easier to tell with sniper rifles. I have had shots on stationary targets at longer ranges where the shot land 2 or 3 mil dots to the left or right of the target like the was some sort of balistic random deviation out side of scope drift... also have had issues of some objects like rocks deflecting shots even though visually the target was visable around it
I am on the Ps4 and I notice this a lot with the G28. My stats might show that I have been doing decent with it. However, it is a struggle to trust this sniper rifle. I love its looks but maybe Ubisoft needs to look at its scope reticle. The SR-25 seems to be the best in terms of precision and accuracy. Good lord, when I got it at Caimanes, I did the missions nearby its location, and it's just a sniper's dream... Only thing which you need to be careful, is its noise because you do need to maintain a farther distance even with a silencer compared to other sniper rifles. Mk14 is also my other preference along with the SVD. I also play on extreme diff so it is super critical to do everything close to inch perfect.

Happened to me tons of times in the beta (ps4). Even with still targets and dead on aiming my shots would do absolutely nothing. My first thought was, ok they've adjusted the bullet drop from the main game, but after messing with it a bit more I had the same results.
I've tried the PvP twice since the Ghost War came out. Got so bored by the waiting times and I'm so sick of reporting bugs (some of them still unfixed) across all modes since day 1, that I've decided to move along and play something different.
