Here's a video of me getting wrecked haha. But it looks like the killing blow should have been blocked. There seems to be one frame right before the hit where the guard disappears and becomes black, but I did not do any action other than block. Either this is a bug, or my controller sent a phantom light attack, guard break, or dodge without me pressing the button.

I can't really tell you for sure either because it seems like you moved to block, but it's possible that the attack had already gone through so you may have just been a teeny bit too slow. It also could have been latency, or lag. Or a few other possibilities.
If you can, send in a report and we can have some more knowledgeable eyes take a look.

Yup, if you open the video in youtube and slow it down, you can see, that he puts the block perfectly from the timing, but doesn't decay but just disappears ^^

This bug is known all over all assassins, every assassin knows it and hates it, many player mentioned it since release and maybe those who played the alpha/betas before us.
