XBOX ONE. For five days I get the error message MIKE C-25-414 "Your account is not available at the moment."
Of course, all these details are 100% correct: port forwarding open NAT static IP DNS google GOLD LIVE subscription renewed.
Through the technical support service I performed the following procedures: router and modem restart xbox cache cleaning re-install the game re-install xbox account reducing the number of friends from 93 to less than 30.
Further tests I made:
1) Loading my xbox profile on the console of a friend (who plays quietly the game with his profile) I still receive the error MIKE C-25-414. 2) Loading the profile of a friend on my console can load and play with his character.
It seems as if the data of my game progress are broken, definitely not a problem due to my internet connection or network settings. It could be a problem with the connection between the xbox account and the Ubisoft account. I honestly do not know what else to try. Tomorrow I'll call again the customer service and I will update you.

I had the exact same MIKE C-25-414 error on PS4 for 6 days which made my game totally unplayable as the game wouldn't log in. However, after the server maintenance took place today (Detailed here: http://forums.ubi.com/showthread.php/1629913), my game will now log in and it's playable again. This may have also fixed your issue so have a try and see if it will log in.

I contacted both support services, Ubisoft and Microsoft.
With Ubisoft, we quickly realized that it was not a problem of my internet connection, not even of their servers.
We tried everything, reset the Xbox, delete the game and the saves data, even the whole game, but it changed nothing.
It gave us the impression that my two accounts, Ubisoft and Xbox, wasn't exchanging the authorizations to allow me to download the game progress.
So we decided to contact Microsoft to check this fact.
After several communications with microsoft, who did several attempts, the told me to do another try: 1) unlink my Ubisoft account from the Xbox account.
2) to create a new Ubisoft account and connect to Xbox account.
3) try to start the game.

Unfortunately this doesn't solve the problem.
So I decided to reconnect my first Ubisoft account to my Xbox account, I tried to start the game but the issues was still there.
After four hours, I was keep trying to start the game, until suddenly started! I did not do any operation immediately before that moment, I assume that the operation recommended by Microsoft was the decisive one and that it took four hours to a possible refresh of the account.
