With that statement, I think GRW can create a new setting and story using the same control mechanics large enough to be a stand-alone expansion. The story could be to turn these self-defense groups to good and have them work together with the government. So it would be like each area within Guerrero is a city within this one-province expansion (instead of a whole country and the countries provinces). The main opposition can still be Santa Blanca, as they do originate from Mexico and also have gang members receiving the exports in these areas.

An added dynamic could be whether or not the mission succeeds/fails the city would become either forever hostile with the city's self-defense force turning on the Ghosts or the city's self defense force decimated by Santa Blanca and now the city under the gang's control. "Winning" each city would be a special reinforcement holding R1 as a limited usable resource. Each City being a different resource and maybe multiple cities. If the game doesn't have enough content from there. Each resource could have multiple cities to increase it's level. The final mission could be to take over the Santa Blanca's HQ in Guerrero after at least one city of each resource is complete by either losing or winning control. Winning would cause the SB to force themselves out seeking to retreat. Losing could be revealed by SB becoming too confident and revealing their position. Programming wise, you could make the area previously inaccessible like how the Division had the incursions.

Depending on how meaty they want the final mission. There could be 3 HQ's each with their own boss. I don't currently have ideas for bosses but maybe a MGS3 The End style, sniper fight would be cool. Include some sort of tracking element in-game as well. But that might be too ambitious.
