When your logged into the game, go to your profile, click info, there should be a tab with your items.

I saw in other thread that 16 digit code is for PC.

I have same issue with my PS4 - 12 digit code. I received an email from Ubisoft with link to click it to my PSN account for redeem. I entered it, but it was invalid... not sure why.
