I don't know of this is a bug with the new patch. Or something is wrong with my save file but when I beat the samurai campaign on Realistic it didn't give me the rewards that come with it. Not sure if a Dev or admin will see that. But I'd just like to make it known.
Id also like to bring it to your attention that the team colors are also bugged I think. Dusk and Storm are locked out for me, and they're both available at level 1. rep 0.

Okay, just an update. I fixed the problem. It was that Orochi vs Warden mission. What happened last time is I died and I killed the warden at the same time. And it counted it as not completed. It fixed it upon finishing the mission though. Giving me the items i didn't have.
Dusk and Storm colors are still bugged though.
