I've had a similar problem on Xbox. Usually it's after I finish an order am go to take it off. After I remove the finished order I cannot start a new one. What I do to fix this is I press the Xbox button on the controller. This takes me to the main Xbox screen. Then I press the start button while"for honor" is selected. This brings up the games menu interface. Then I go down to quit. This basically resets the game. After that I'm usually able to re-equip orders.
