I'm afraid I don't have a lot of information on what might have caused this bug, but in playing the game I encountered an instance wherein my character would not respawn.
The incident occurred after I had waited for the respawn timer to tick down and hit the button prompt once it was over. The zoom-in animation, the one where you move from the bird's-eye down to your character as it respawns, began and just as it started the host of the game left (or disconnected). This caused the game to shift who was host and reestablish connections. Once this had finished, the zoom-in continued, except when it finished, there was no model for my character. In addition, I was unable to move around or perform actions, outside of rotating the camera. I could access the menus, but I could not perform any actions in game, such as attacking; I could tell it wasn't just the model was missing, because no sounds played when attempting to attack or such either.
It also did not just occur in my instance of the game; I was playing with a friend at the time, and he was unable to see or affect my character either. He tried hitting the area my character should be in, but there were no collisions of the models.
I'm sorry there isn't much more information I can give, but I hope that letting you know the events that led up to this, you might be able to figure out what went wrong, or at least replicate it.

I was playing a round of domination my team was winning then we had everyone on my team except myself disconnect and be replaced with AI. I had died at zone C and had tried to respawn from there right before everyone disconnected but I ended up losing my character model. Apparently it dissipeared but it ghosted at checkpoint C saying that the point was contested. http://imgur.com/gallery/KamEv These are my screen shots of the incident, all buttons did nothing.
