If the Controller shuts off or disconnects during a match and if you reconnect it, Once you are locked on a hero, right trigger will switch targets and not use the heavy attack.

Heavy still works outside of lock, but once you lock it goes into target swtich.

Only way I found to fix it is to shut down the game and restart.
