I finally got the three months thanks to Dennis H from support! Those who are having this problem, open a case at Ubisoft's main support site and attach a screenshot showing proof that you purchased the Gold Edition from Xbox Live. Just a simple screenshot the game in your transaction history from xbox.com will do.

I bought JD 2017 Gold Edition (Digital version) + Just Dance 1 year pass for PS4
Currently, at the top right corner, my pass have 461 days: 365 days (1 year subscription) + 90 days (Gold edition) + 6 days (Game gave me for free)
So far so good, but when I go the JD Unlimited Submenu it only show me two options:
1) Extend my subscription
2) Enter the 3 month code that comes with the Gold Edition (disc)
So, I cannot access to the JD Unlimited song library...
Never played JD 2016 on PS4, btw

I, a couple of weeks ago, downloaded Just Dance 2017 Gold Edition from the Nintendo e shop on my Wii U. The game came with a 3 month subscription to Just Dance Unlimited. This worked for a day or two, then stopped. I never received a code or anything and I really want my 3 month subscription.
Can someone either contact me or tell me how to fix this?
