I had an issue where no matter how many times I viewed, added, changed a piece of equipment on the Warden I still had the i icon as if I had not viewed it. I don't know how many people had the same issue, but the Warden was the only character I had an issue with this on.
i had this happen on a few characters, it can happen, if between games you dismantle a new bit of gear to improve and old one, but when you go back to the main menu it says you have a new bit of gear, but you can't select it to get right of the new icon because you have already destroyed it.
