How do we detect and/or report cheats that can not be 100% determined and recorded?

I am not sure if everyone is aware of all of the functionality hacking/cheating software offers and i am not going to list it all (I am sure Ubi/Massive knows it and listing it may tempt others to give cheating a try) but i have to ask this:

If a person only turns on a very small subset of options like being able to see every NPC and player in the zone and/or having unlimited ammo - how do do we a) determine if the person is doing it and b) report it? You will not be able to really 100% tell if that player or group just happened to find/stumble upon you or if he/they were tracking you the moment you entered the zone. Neither will you be able to see that while he/they do stop to reload - their ammo count isn't really going down.

Aside from automatic anti-cheat detection such cheats are impossible to reliably detect and prove in order to report. What are we supposed to do about it then?

Before anyone says something along the lines of "what do you care if they do it?" - seeing everyone at all times is a huge tactical advantage in PvP and not running out of ammo is huge time advantage in PvE as well as an advantage in prolonged engagements in PvP. IMHO this is the biggest cheat threat out there, not the teleporters or people using aimbots/massive ROF but those using subtle cheats like i described as there is no real way of knowing... unless you are running a cheat yourself and you can see everyone and can determine a pattern of movement indicating that they see you as well.

As i mentioned in my op - how can anyone actually know that this is in fact happening? Your group is suddenly engaged by another group - just your average engagement or did they track you down using cheats? Group running around killing things - do they have to restock or are they using the unlimited ammo cheat? No way to tell unless someone has any other ideas besides reporting on a guess as that would just flood the reporting system.

No way to know, to be honest. It'd take a GM to spectate them and look for patterns, but that's not available right now.

I was also wondering about this. Most hacks are pretty damn obvious and they one-two shot you with any weapon.
But what if some of them crank up the damage to 2x or 3x. Instead of 100x...
We can't tell... Can the admins or the cheat system detect this? Well I do hope so.

That's one of the reason "second chances" doesn't work well. They get caught, they just come back and be more "careful".
