hacking on consoles is fairly common. many games have suffered notoriously for it. gtav is the biggest one that springs to mind in recent years
and if the division does run trusted client on consoles that would be easy to hack as well.
just be a case of connectiong your console to the internet through a pc and modifying packets that pass through. so your xbox sends a packet saying you fired a 10k bullet the pc catches it and changes it to say that bullet did a million damage and then passes it on to the game server. which would then accept it as legitimate due to the "trusted client" infrastructure
the only reason its not as prolific as the current state of pc hacks is that it's not simply a case of buy some crappy program that does the hack for you. which generally means most of the pc hackers have zero skils they just run a program / script someone else made.
