Sooo... I go to jump over a wall and I get stuck. Can't jump back or forward or down. I can just run along this little bit of wall. AND I CAN'T FIND A MENU OPTION TO RELOAD.
God, after losing server connection right as I was about to finish a boss to weird glitches all over the place... and now the game won't let me get off a silly little wall. Destiny is a winner all right... winner of most confusing menus. Well thought out, they are not (and the mapping is even worse).

I can't remember the last time I got stuck in the map except in the underground where it happens just enough to remind me. I do know of one place on the map that I used to get stuck in, now I just don't go to that spot. Interestingly, I do occasionally get safehouse doors that won't open. It happens the most in the safehouse where bulletking is. At least they fixed the griefing of people intentionally standing in doorways preventing people from coming in or going out which was a thing for the first couple/few weeks on release back in March 2016.
Along with what echolecter said, you can also log out and back in. If you have friends online when it happens, I believe you can also join their group, then leave it again (if desired). I am pretty sure this works in combat.
