So i was wondering, the game obviously put a lot of attention on NY, but is it "Just" NY that got frakked or the whole world?
I came across some throw away lines, in origin, about outbreaks elsewhere. But again judging from that same source, there's still enough of a US government presence for the Prez to sign the division into action and the news stations are still broadcasting up until the emergency broadcast starts and the watch lights up.
But on the other hand, what would be the point of going postapocalytic maniac if there is still a controlling outside of NY. I mean you run rampant for a while, but eventually the USMC, in full biogear or vaccined up comes knocking down your stronghold gate. And if they find flamer fuel near you, you're probably frakked and not in a good way.
Anyway, just me wondering.
With how international and national travel are now days it has most likely spread outside of New York City. It would depend on how fast the containment of New York City under quarantine and the ban on national and international travel was put in place. There would be a very high likely hood that it would turn into a Global Pan epidemic which could bring most of the world to a halt.

Looking at JFK airports common large scale flights, I could see the outbreak reaching London, Los Angeles, Italy, Germany (based on average passenger destinations)
So yeah, it could EASILY be a worldwide epidemic. Im kinda hoping/anticipating that you find out about more major outbreaks by the end of thr storyline leading up to a sequel.
Heard the same on the pirate radio station in the DZ safehouse. Apparently every major metropolitan area in the US has an outbreak. A pandemic usually means "everywhere". The smallpox have an incubation time of around 14 days. You could pay a visit to the U.S., catch the dollar flu and fly home to europe, without knowing, that you are already dead. Yesterday sneeze at the airport and a person on the way to Kuala Lumpur, gets infected as well. The higher the incubation time, the higher the risk of wide, unnoticed distribution.
