If the game suddenly started playing poorly in the last few days then it's possible that something on your machine was altered. Have you installed new programs? Do you have different services running on your PC than you had previously?

I had the exact same thing happen to me this weekend and did not make any changes to my system. The only "change" I made was I installed Metro 2033 on steam, and finished the game over the weekend with awesome performance and no issues while Trials ran like complete garbage the few times I tried to play it. Multiplayer has never worked for me but now I can't even play offline because the frame rate has suddenly become unplayable.
