What I've noticed is this: You turn CP to level whatever. You fast travel or start up the CP. 2 things can happen: Civilians start a take over witch puts it to normal (even tho you already started it), or when you use the Flair Call, the game bugs out and does what I just previously said. This is what I've noticed to cause the CP restarting over.

I don't think that's it. I play solo 99% of the time. And if the CP in question has issues with me traveling between maps 10 minutes after I start the CP fight(that's how long it took to take over), then that's a SERIOUS server-side issue that would happen more often than the once in thousands of hours that this issue pops up.
The yellow box you opened has 4 items but a Control Point 4 yellow box has 5 items. Your world is on Heroic so you "should" always have level 4 CP.
I bet it's a bug. The box you opened should spawn in a world on Challenge CP 3 or whatever level it is that has a main loot box with 4 items like you received.
I did a quick CP this morning to see how many items are in a CP 4 and it's 5.

Something is definitely happening...
Last night I joined a group doing open world stuff, we were going for CPs and anything that was in the way. World was set to heroic with three directives.
At some point when were just en route to a CP, I noticed the world label changing... It did at least one full cycle between heroic, normal, hard, challenging and back to heroic in a matter of ~10 seconds.
Nobody was messing with the settings.
Is it happening more often lately? Haven't seen this before. And there seems to be more reports of this bug lately.
Had this happened to me before CP4 and upon clearing the area and opening the chest yep LVL 1 drops, can't understand why they didn't bother checking or looking into this issue or bug. If there were bugs reported dev's should be making an announcement or notice on what is happening but by the looks of it they do not care anymore.
Heroic should be giving us better rewards or loot not level 1 rewards or loot and it just defeats the purpose on farming in Heroic difficulty only to get less rewards from it.
I saw things get even weirder today. My teammate and I finsihed up a Heroic control point in a Heroic world and were informed that the storeroom was unlocked, but enemies from an elite resource convoy that had wandered into the fray still needed to be finished up. While I was taking care of much of that, my teammate went over and got his Heroic loot. Shortly after that, however, we noticed that the "press F to call allies" was back, the CP was now incomplete, and when I ran over to check the storeroom the loot box was not available to me. And the HUD was telling me that this was now a Normal level CP.
So we called the allies, finished up another wave or two resulting from that, and I finally got my loot, but it was three purples and a poorly (~25%) rolled gold item, so obviously Normal difficulty rolls.
This was fortunately only a mild annoyance in the end since plenty of stuff was dropped by enemies before the CP converted to Normal difficulty, and my teammate also gave me what he'd found of the brand gear I was looking for. But still, odd!
