You can get up to about 50 by playing NG+.
No NG+ so far. Must have been a reward for a mission. Well I will find out on a new playthrough. (I am also sure it is not the "hoarder" fighter, he extends the capacity of ammo and throwables, but only as long as he is assigned.)
It's likely your Gun for Hire. If you dismiss them, you won't benefit from their perks unless they are following you again.

Will revive Fangs for Hire- Back Up
Will revive themselves when downed.- Quick Recovery
Will take less time to recover health.- Field Surgeon
Will fully revive your health when downed.- Mechanic
Will repair the vehicle you drive.- Hoarder
Will allow you to carry more ammo.- Fortitude
Will draw attention and take more damage.- Herbalist

Do you currently have a Gun for Hire who has that perk? Did you somehow cause a glitch to make it permanent or joined a co-op session while they have a Gun of Hire of their own who also has the same perk?
