Ok, so this issue isn't what I originally thought, but it's just as bothersome, and should definitely not be happening. There's most certainly a bug in this game. It'd be nice if someone could attempt to duplicate the issue for me, though..
The stolen focus is actually happening because the game is somehow allowing the mouse cursor's location to "bleed" outside of the game window onto an extended monitor that FarCry4.exe isn't utilizing, *even* in fullscreen mode. This is a big no-no from a design standpoint. I'm not even sure how the game is managing to do this.
In my case, the extended monitor is located to the right of my main monitor where the game window is displayed in full screen, and only on that screen. So, every time I click a mouse button (usually "Aim") while moving the mouse to the right, somehow the click is being registered outside of the game window, hence why the focus is stolen. It doesn't have to be abrupt movement, either. It can be small and relatively slow, as long as its in the direction of the extended monitor. To test this, I switched the extended monitor's location to the left of my main monitor, where the game is displayed. Lo and behold, now whenever I click a mouse button while moving the mouse to the left, the window focus is stolen.
The quick and dirty fix is to disconnect the extended monitor via desk.cpl, but I shouldn't have to disconnect my monitor just for this game to work properly. Somebody needs to fix this..

The title describes the basic issue. The "Aim" key causes the game window to go from full screen to windowed, and when it does, it INACTIVATES the game window (greys out the title bar) and the game stops accepting input. Every time this happens, I have to Alt-Tab out of the game, and reactivate it from the taskbar. By that time, chances are I'm dead, because when are you most likely to use the "Aim" key??? In the middle of an effing firefight!
The game is unplayable because the issue will occur multiple times within a 10-minute timeframe. By "in certain scenarios", it appears that the issue has the propensity to occur when the aiming reticle is located in the immediate vicinity of a non-terrain object (e.g. vehicles, A.I., etc.). The object can be in front of or behind a terrain object; it doesn't matter either way. Also, the aiming reticule doesn't have to be directly on the object either. There seems to be a transparent 50-70 pixel "zone" surrounding the object in all directions where the issue is also likely to occur. Please note that this condition is not exclusive, per se. However, the issue does seem more likely to occur under these circumstances.
I've done my due diligence. This is not a mouse issue or a keyboard issue. I've swapped out both, and it doesn't change a thing. I've changed the key bound to "Aim" ( using different mouse and keyboard binds), and it doesn't matter what the Aim key is bound to. It's not programs in the background, either. I play first-person shooters left and right (Far Cry 3 among a hoard of others) with the same key binds and the same hardware, and this has never, EVER happened before. As a precaution however, I did try closing a lot of the non-essential system tray apps. I even tried disabling Setpoint and Logitech Gaming Software. Alas, this didn't change a thing, but that's not the least bit surprising.. This is definitely pointing towards an issue with the game itself, and I would really appreciate some assistance from Ubi's support staff..

Logitech Setpoint is version 6.65.62, Logitech Gaming Software is version 8.57.145, and Far Cry 4 is obviously patched with whatever version uplay wants to throw at it.
Can anyone else corroborate this issue? Surely, I can't be the only one having it.. Would someone from Ubi be so kind as to send this issue up higher? As it stands, this game is not playable by any reasonable definition..*/

I can confirm that I see the same issue on Far Cry 4 -- I'm also running two monitors. My hardware setup is similar to the original report of this issue:

Usually at the height of action the mouse focus is lost by Far Cry 4, and I'm frequently dead before I can restore the mouse focus. This does make for a higher degree of difficulty, but is quite annoying.
